Care Coordination

Population Health case managers assist high-risk patients and those transitioning from hospital to home, or a community-based post-acute setting, by providing disease-specific education, coaching on self-management skills and offering support to under-resourced patients negatively affected by social determinants of health.

Wellstar Health System is the only health system in Metro Atlanta to be selected as a member of the Mayo Clinic Care Network. As part of the Wellstar Clinical Partners network, complimentary consultations are available with a Mayo physician for additional opinions on complex cases, at no cost to the patient.

Partners also have round-the-clock access to Mayo Clinic-vetted medical information and guidelines through AskMayoExpert, a web-based provider resource. Mayo Clinic shares a common philosophy with Wellstar – an unwavering commitment and mission to improve healthcare delivery. Working together, we hope to ensure the best care and treatment plans for even the most complicated diagnoses.
